Ans:-Reciprocal registration means transfer of registration from one state council to another state council. It requires all registration related documents mentioned in the stipulated form to apply and a no objection certificate from the state council where he/She registered his name earlier as per Sec. 32(2)of the Pharmacy Act, 1948 (VIII of 1948).
Ans:-Address may be changed at any point of time collecting a Address change Form.But he or she has to produce Voter card, Addhar Card or Passport as a proof of his recent address.
Ans:- Duplicate registration certificate can be issued to a registered Pharmacist if he or She comply all the documents mention behind the duplicate Form.
Ans:-Damaged Registration Certificate can be issued to a registered Pharmacist who comply all required documents mentioned behind the stipulated Form.
Ans:-Surname of a registered Pharmacist can be changed if he/she comply all the documents required for the purpose such as Voter Card/ Adhaar Card/Passport and marriage registration Certificate and an affidavit mentioning the cause of changing Surname.
Ans:-A registered Pharmacist can incorporate any additional qualification complying all the documents such as Marksheets certificates alongwith a prescribed Form issued for additional qualification.
Ans:- A registered Pharmacist under West Bengal Pharmacy Council can transfer his/her registration to any other state council of India if he or she apply to any state council first then a no objection certificate is required from the state council where he is registered with.
Ans:- All kinds of Form can be collected from the office of the West Bengal Pharmacy council within 10:30 am to 2:30 pm.
Ans:-A registered Pharmacist can enroll his/her name with the xerox copy of original Certificate and renewal certificate and the requisite fees required for the purpose.
Ans: A UGTP certificate will be valid for five year from the date issuance of the certificate.
